Prevalence of co-morbidity increases among patients and other factors
The prevalence of multiple chronic conditions such as diabetes, obesity, hypertension, kidney and heart disease among individuals increases. Patients will require more complex care, placing extra strain on the system as the need for specialist skills, as well as multidisciplinary and integrated working increases.
