People with long term conditions self manage and self refer
People with long term conditions self manage and are provided with the knowledge and skills they need to manage their conditions more confidently and to make daily decisions which can maintain or enhance their health and wellbeing. There is significant scope for new and emerging technologies to help people to self manage where it is safe and appropriate to do so. Equipment is used by people in their homes to monitor their vital signs or to monitor their condition remotely by healthcare staff (who can make a positive contribution to helping people manage their condition more effectively).
Proposed workforce impact
- It reduces demand in acute wards.
- Staff are required for the development and delivery of education and training programs required for independent, voluntary and community organisations.
- Trainers should be provided to ensure those delivering self-management support have the skills needed to do so.
- Staff are required to deliver self-management support.
