People with complex, chronic needs, and their relatives, demand integrated care

The DH, patients' organisations and professional bodies have all idead statements that people living in the community with complex long-term health and social care needs should receive integrated care. They all agree that currently this often does not occur. The demand for improvements in the coordination of care will become louder and more urgent over the next 20 years. This may affect nursing numbers, skill mix, approach, and locations.
