Hot clinics are the norm

To reduce the need for unplanned admissions and improve discharge processes, so-called hot clinics and management plan models are developed and set up. These hot clinics provide patients with planned assessments including complete access to a team of specialists. The patient leaves the assessment with a management plan for their care, allowing increased elective work before health deteriorates and an acute setting is required. This results in less acute admissions and improves continuities and efficiencies post-discharge where health and social care services are more integrated as a result of more joined-up planning at the service level.
